In this session, we will cover key steps and procedures for conducting an effective incident investigation with steps in place to ensure key findings and corrective actions are completed. We will discuss the different incidents and degree of investigations necessary for each incident type as well as the importance of working in sequence: incident scene integrity, witness interview, fact finding, direct cause analysis, root cause analysis, risk assessment, corrective action assignment and track to completion to close the loop. Thorough investigations also include policy and training effectiveness reviews, we will cover tips and best practices for effectively reviewing education competency. Too often, organizations fall into the trap of re-training employees without proper evaluation of a knowledge gap. All ethical organizations desire ZERO, we all need to be pre-prepared to learn if and/or when incidents occur so we can continue to provide a desirable healthy and safe work environment for all employees to thrive.
